Purpose
To quantify intra-device repeatability and inter-device agreement of corneal astigmatism magnitude and axis, and steep keratometry (K2), across four corneal tomography platforms in healthy eyes.
Setting
Research Department, Oftalmosalud Institute of Eyes, Lima, Perú.
Methods
Prospective, cross-sectional reliability and inter-device agreement study. A single examiner acquired three consecutive scans per eye in randomized device order using a rotating Scheimpflug camera (Pentacam HR), a Placido–Scheimpflug hybrid (Sirius), a dual rotating Scheimpflug–Placido system (Galilei G4), and a spectral-domain AS-OCT with Placido topography (MS-39). Primary outcomes were corneal astigmatism magnitude (D) and axis (°). Repeatability was summarized with within-subject SD (Sw) and repeatability coefficient (RC=2.77×Sw). Inter-device agreement used ICC and pairwise differences; Bland–Altman was applied to scalar parameters.
Results
Ninety-six eyes of 48 healthy participants (mean age 32.23±6.10 years; 64.6% female) were included. Intra-device repeatability for astigmatism magnitude was Sw 0.127–0.277 D and RC 0.353–0.768 D. Between devices, differences in astigmatism magnitude showed RC 0.456–0.732 D, whereas axis differences were larger (K2-axis RC 10.786–18.942°). Anterior K2 showed high repeatability (RC 0.312–0.540 D) and excellent inter-device reproducibility (ICC 0.995, 95% CI 0.979–0.998). Inter-device ICC for anterior astigmatism magnitude was 0.992, while axis ICCs were lower (K1 0.918, K2 0.912; all p<0.001)..
Conclusions
In healthy eyes, corneal astigmatism magnitude and steep keratometry are highly repeatable within devices and reproducible across platforms, whereas axis orientation shows larger between-device variability.
