Purpose

To directly compare visual performance, refractive predictability, and optical quality after implantation of a hydrophobic segmental refractive IOL (ACUNEX Vario) versus a hydrophilic-based segmental refractive IOL (LENTIS Comfort LS-313 MF15).

Setting

A single-center observational comparative cohort study

Methods

This single-center, observational comparative cohort study included two cohorts: Group 1 (G-1), prospectively enrolled eyes implanted with ACUNEX Vario; and Group 2 (G-2), a historical cohort implanted with LS-313 MF15. Examinations were standardized with a nominal 6-month endpoint. Outcomes included monocular and binocular visual acuity (distance, intermediate, and near), refractive accuracy, monocular defocus curves, mesopic contrast sensitivity, and retinal image quality at 3 months measured by pyramidal aberrometry.

Results

A total of 163 eyes (111 patients) were analyzed (G-1: 66 eyes/35 patients; G-2: 97 eyes/76 patients). Monocular UDVA, UIVA, CDVA, refraction, DCIVA and DCNVA were comparable between groups (P>0.05). Near vision favored G-1 (UNVA/CNVA, P<0.05), and binocular acuity (uncorrected and distance-corrected) was better in G-1 at all distances (P<0.05). Defocus curves and mesopic contrast sensitivity were similar. Refractive accuracy within ±0.50 D and ±1.00 D was higher in G-1 than G-2 (74%/97% vs 60%/88%). In the optical-quality subset, pyramidal aberrometry (RMS/HOA, PSF Strehl) did not differ (P>0.05). PCO and enhancement rates were low in both cohorts.

Conclusions

ACUNEX Vario and LS-313 MF15 provided comparable, design-consistent distance-to-intermediate functional vision. ACUNEX Vario showed advantages in strict refractive predictability and near vision, with more favorable binocular performance overall.
