Purpose
To objectively quantify the depth of field gained by patients undergoing PRESBYOND® Laser Blended Vision. Conventional outcome reporting relies on visual acuity and subjective defocus curves, which are inherently prone to variability and patient bias. In this study, we report objective depth of focus measurements derived from ray-tracing wavefront analysis to determine the true magnitude of functional range expansion achieved. Secondary objectives included assessing refractive predictability and correlating refractive accuracy with objectively measured depth of field
Setting
Single-centre, tertiary referral eye care institution specializing in cataract and refractive surgery, Uma Eye Clinic, Chennai, India.
Methods
Prospective interventional study including 110 eyes of 55 presbyopic patients (60 eyes/30 hyperopes; 50 eyes/25 myopes) aged ≥42 years undergoing PRESBYOND® Laser Blended Vision. Inclusion criteria were presbyopia with clear crystalline lens, stable refraction, normal corneal topography, and ocular health suitable for laser vision correction. Follow-up was performed at day 1, 1 week, 1, 3, and 6 months; six-month outcomes are reported. Standard visual and refractive outcomes (nine standard graphs) and objective depth of focus analysis using ray-tracing aberrometry were evaluated.
Results
At 6 months, in the myopic cohort, 100% achieved binocular 20/20 distance and J5 near vision, with 61.5% achieving 20/20 and J2. In the hyperopic cohort, 100% achieved 20/25 and J5, while 81% achieved 20/20 and J2. Refractive outcomes demonstrated high predictability across both groups with minimal residual spherical equivalent. Monocular objective depth of focus increased by 0.5 D following treatment. Binocular depth of focus demonstrated a continuous, usable functional range between 3.16 D and 4.02 D in both cohorts, confirming meaningful expansion of depth of field without compromise of distance visual acuity.
Conclusions
PRESBYOND® Laser Blended Vision provides predictable refractive outcomes with excellent binocular distance and near visual performance in both myopic and hyperopic presbyopes. Objective analysis demonstrated a measurable increase in monocular depth of focus and a broad, continuous binocular functional range exceeding 3 D. Unlike subjective defocus curves, objective depth of focus assessment offers reproducible quantification of functional range expansion. These findings provide optical validation of the blended vision concept and support its effectiveness in achieving extended functional vision without compromising distance acuity
