Purpose
To assess the clinical efficacy of reproxalap in inflammatory eye diseases.
Setting
A systematic review and meta-analysis
Methods
This systematic review is registered with PROSPERO (CRD420251070669). A search of PubMed, Embase, and ClinicalTrials.gov was conducted from January 2000 to June 2025, to identify studies evaluating reproxalap in inflammatory eye diseases. Study quality was assessed using the Cochrane Risk of Bias tool (RoB 2.0). Pooled analyses were performed using RevMan 5.4 with a random-effects model.
Results
A total of 14 RCTs (n = 2,692) were included: 8 on dry eye disease (DED) (n = 1,888), 4 on conjunctivitis (n = 637), and 2 on anterior uveitis (n = 167). In DED, reproxalap significantly reduced conjunctival redness (SMD: -0.29; 95% CI: -0.45 to -0.12; p = 0.0005), improved Schirmer test scores (SMD: 0.38; 95% CI: 0.25 to 0.50; p < 0.00001), and reduced ocular dryness scores (SMD: -0.20; 95% CI: -0.37 to -0.02; p = 0.03). In allergic conjunctivitis, reproxalap significantly reduced ocular itching (SMD: -0.38; 95% CI: -0.51 to -0.25; p < 0.00001), ocular tearing (SMD: -0.30; 95% CI: -0.51 to -0.10; p = 0.004), and conjunctival redness (SMD: -0.42; 95% CI: -0.64 to -0.20; p = 0.0001).
Conclusions
Reproxalap effectively reduces key inflammatory symptoms in DED and conjunctivitis and shows potential as a corticosteroid-sparing agent in anterior uveitis.
