Purpose

Impact of Capsular Tension Ring (CTR) Implantation on Intraocular Lens (IOL) Stability and Posterior Capsule Apposition: An iOCT and AS-OCT Study

Setting

Narayana Nethralaya Superspecialty Eye Hospital, Bangalore, India.

Methods

This prospective study included 251 eyes undergoing phacoemulsification. Intraoperative spectral-domain OCT (Rescan 700) was performed in 51 eyes to measure the vertical distance between the posterior capsule and IOL before and after CTR implantation using 11, 12, and 13 mm CTR sizes. Postoperative AS-OCT was performed in 200 eyes, divided into two groups (100 with CTR; 100 without CTR), to assess horizontal and vertical IOL tilt and decentration.

Results

CTR implantation significantly reduced the IOL–posterior capsule distance intraoperatively (p < 0.001), with the most substantial reduction observed with 13 mm CTRs. Postoperatively, AS-OCT analysis revealed no statistically significant differences in horizontal or vertical IOL tilt or decentration between the group with CTR and the group without CTR.

Conclusions

CTR implantation significantly enhances immediate intraoperative IOL–posterior capsule apposition, with larger ring sizes providing the most effective contact. Postoperatively, AS-OCT demonstrated that eyes with CTRs achieved excellent IOL stability, with tilt and decentration profiles that were equivalent to those of standard surgeries. These findings suggest that CTR implantation successfully facilitates a stable capsular environment and anatomical outcomes comparable to routine cases. The use of iOCT and AS-OCT provides a robust, objective method for monitoring these critical parameters throughout the perioperative period.
