Purpose
To evaluate the long-term efficacy, safety, and morphological changes of lenticule transplantation for hyperopic residual refraction following LASIK (Laser in Situ Keratomileusis).
Setting
Eye Hospital, Prishtina, Kosovo
Methods
This study included 84 eyes of 42 patients (32.9 ± 7.5 years; 22–44 years) with post-LASIK hyperopic residual refraction (+1.00 to +6.00 D) and astigmatism (+0.75 to +2.50 D). Donor lenticules were obtained from 20–29-year-old myopic SMILE patients (thickness 88.4 ± 28.1 µm; −4.42 ± 1.40 D; optical zone 6.60 ± 0.15 mm). A 140-µm intrastromal pocket was created without lifting the original LASIK flap, and lenticules were transplanted targeting −0.50 D. Postoperative assessments included UDVA, CDVA, slit-lamp examination, corneal topography, pachymetry, and anterior segment OCT. All patients completed 5-year follow-up.
Results
No cases of rejection, intraoperative or postoperative complications, or surgical revision were observed.
At 5 years, UDVA improved from 0.63 ± 0.21 logMAR to 0.03 ± 0.03 logMAR. UNVA improved from J7 to J2 at 40 cm and from J6 to J3 at 80 cm.
Central corneal thickness increased from 490.0 ± 33.8 µm to 572.3 ± 27.1 µm. The spherical equivalent decreased from +3.95 ± 1.40 D to −0.40 ± 0.13 D. Keratometric values increased from 41.52 D to 44.27 D for K1 and from 42.87 D to 44.98 D for K2. The Q-value shifted from −0.43 to −0.79.
All changes were statistically significant (p < 0.001).
Conclusions
In post-LASIK hyperopic residual refraction, lenticule transplantation demonstrates long-term safety and efficacy, enabling controlled modulation of corneal asphericity with sustained enhancement of distance and near vision.
