Purpose

The Vivinex Gemetric Plus (XY1-GP and XY1-GPT2-6; HOYA Surgical Optics) is a novel trifocal intraocular lens designed to optimize uncorrected near visual acuity. Although the Gemetric Plus shares the same focal distances as the Vivinex Gemetric (XY1-G), the Gemetric Plus differs in light distribution and is frequently paired with the distance-emphasizing Gemetric in clinical practice. This study aimed to evaluate postoperative visual outcomes following bilateral implantation of the Gemetric Plus.

Setting

Omiya Nanasato Eye Institute, Saitama, Japan

Methods

This prospective study included 54 eyes of 27 patients (mean age, 66.5 ± 6.3 years) who underwent femtosecond laser–assisted cataract surgery with bilateral implantation of Gemetric Plus (XY1-GP and XY1-GPT2–6). The distribution of implanted IOL models was as follows: XY1-GP in 22 eyes, T2 in 13 eyes, T3 in 13 eyes, T4 in 5 eyes, and T5 in 1 eye. The following parameters were evaluated at 3 months postoperatively: uncorrected and corrected distance visual acuity (UDVA and CDVA) at 5 m; uncorrected intermediate visual acuity (UIVA) at 60 and 80 cm; uncorrected near visual acuity (UNVA) at 40 and 30 cm; defocus curves; contrast sensitivity (CSV-1000, VectorVision); and dysphotopsia scores (QoV questionnaire).

Results

Mean postoperative spherical equivalent and cylinder were -0.34 ± 0.16 D and -0.22 ± 0.31 D, respectively. Binocular UDVA was 0.05 ± 0.01 logMAR; monocular UDVA/CDVA were 0.08 ± 0.01/-0.06 ± 0.05. UIVA (80/60 cm) was 0.15 ± 0.01/0.13 ± 0.09. Binocular and monocular UNVA (40/30 cm) were 0.09 ± 0.01/0.08 ± 0.02 and 0.05 ± 0.16/0.08 ± 0.01 logMAR, respectively. Photopic contrast sensitivity was 1.53 ± 0.22, 1.69 ± 0.23, 1.32 ± 0.09, and 0.87 ± 0.10 (log units) at 3, 6, 12, and 18 cpd. The distance-corrected defocus curve maintained visual acuity of 0.27 logMAR or better between +0.5 and -3.5 D. All patients were spectacle-independent, and no severe dysphotopsia affecting daily activities was reported.

Conclusions

Gemetric Plus provided good visual acuity at distance, intermediate, and near, including excellent performance at 30 cm, and was associated with a high rate of spectacle independence. Bilateral implantation of the Gemetric Plus may be a suitable option for populations using complex characters, such as Chinese characters.
