Purpose

To evaluate the safety, intraoperative performance, and clinical outcomes of selective laser capsulotomy (Capsulaser) in pediatric cataract surgery, and to develop an age-dependent correction formula for precise capsulotomy diameter calculation in the highly elastic pediatric capsule.

Setting

magrabi eye hospital jeddah ksa

Methods

In this prospective case series, 13 eyes of 10 pediatric patients (mean age 4.10 ± 3.38 years; range 2–8 years) underwent cataracts surgery. . All anterior capsulotomies were performed using the Capsulaser system. To account for pediatric capsular elasticity and subsequent "runaway" expansion, regression analysis was utilized to develop an age-dependent correction formula for laser diameter settings. Safety was assessed via surgical complications and ocular adverse events (AEs) at 1 day, 1 week, 1 month, and 6 months postoperatively.

Results

Capsulaser successfully created circular, centered capsulotomies in all cases, including those with lens subluxation. No intraoperative complications (such as radial tears or incomplete treatments) were reported. Postoperative follow-up to six months showed zero ocular AEs; specifically, there were no instances of excessive capsular shrinkage, retinal detachment, or atypical inflammatory responses.

Conclusions

he use of selective thermal laser technology for anterior capsulotomy in pediatric patients is safe and effective. It provides a level of reliability and centration that is technically challenging to achieve manually in elastic pediatric capsules.
