Purpose

Evaluation of the outcomes of the PEARL-DGS Second Eye biometry formula in patients undergoing bilateral sequential cataract surgery and comparison of this mode with commonly used formulas.

Setting

Patients who underwent bilateral phacoemulsification at the Istanbul University, Istanbul Faculty of Medicine, Department of Ophthalmology between January 2024 and December 2025, received in-the-bag implantation of an Alcon SA60AT intraocular lens (IOL), and were followed for at least one month postoperatively were included in the study.

Patients with additional ocular pathology, a history of previous ocular surgery, or intraoperative complications were excluded from the study.

Methods

Biometric data were obtained in all patients using the IOLMaster 700 device.

Using these biometric data, calculations for the first eye were performed with the Barrett, Cooke K6, EVO, Hill-RBF, HofferQST, Kane, and Pearl-DGS (standard) formulas.

For the second eye, in addition to these formulas, calculations were also performed using the PEARL-DGS Second Eye formula.

The primary outcome measure was the difference (absolute error) between the manifest refractive spherical equivalent (SE) measured at postoperative month 1 and the targeted refractive value.

Statistical analysis was performed using SPSS (Version 26). The Mann–Whitney U test was used for intergroup comparisons. A p-value < 0.05 was considered statistically significant.

Results

A total of 86 eyes from 43 patients (21 males and 22 females) were included in the study. The mean age of the patients was 66 years, and the mean interval between surgeries of the two eyes was 15 days.

In the statistical analyses performed, the difference between the spherical equivalent (SE) calculated by each classical formula and the SE predicted by the PEARL-DGS Second Eye mode was found to be statistically significant (p < 0.05).

With a lower mean absolute error (MAE), the PEARL-DGS Second Eye mode provided significantly closer predictions to the postoperative spherical equivalent of the second eye compared to all other formulas.

Conclusions

In patients undergoing bilateral sequential cataract surgery, the PEARL-DGS Second Eye formula achieved more accurate results in reaching the target refraction for the second eye compared to the other formulas.

The Second Eye mode may provide a more consistent and reliable calculation by incorporating the refractive deviation of the first eye.
