Purpose

To evaluate the refractive accuracy of the ZEISS AI formula (integrated in EQ Workplace) using IOL Master 700 biometry and to assess the clinical performance and intermediate visual acuity of the Lucia 621P "Monofocal Plus" IOL.

Setting

First Eye Center, Seoul, Republic of Korea.

Methods

This prospective clinical study analyzed 285 eyes (182 patients) undergoing cataract surgery with Lucia 621P implantation. Preoperative biometry was performed with the IOL Master 700. IOL power was calculated using the ZEISS AI formula and compared with the Barrett Universal II (BUII) as a control. At 3 months postoperatively, we measured Uncorrected Distance Visual Acuity (UCDVA), Uncorrected Intermediate Visual Acuity (UCIVA at 66cm), and Manifest Refraction (MR). The Mean Absolute Error (MAE), Median Absolute Error (MedAE), and the percentage of eyes within ±0.25D, ±0.50D, and ±1.00D of the predicted refraction were analyzed.

Results

The ZEISS AI formula demonstrated superior precision compared to BUII. The MAE for ZEISS AI was 0.28 ± 0.19 D (vs. 0.32 ± 0.21 D for BUII, p < 0.05) with a MedAE of 0.24 D. The percentage of eyes within ±0.50 D was 91.2% for ZEISS AI, compared to 86.4% for BUII. Regarding visual performance, the Lucia 621P achieved a mean UCDVA of 0.02 ± 0.04 logMAR and a functional UCIVA of 0.18 ± 0.08 logMAR. No significant glistening or dysphotopsia was observed. Contrast sensitivity remained within the normal age-matched range under both photopic and mesopic conditions.

Conclusions

The integration of ZEISS AI technology with IOL Master 700 biometry provides high refractive predictability, outperforming conventional formulas for the Lucia 621P IOL. Furthermore, the Lucia 621P offers extended intermediate vision beyond the range of standard monofocals, establishing itself as an effective "Monofocal Plus" solution for patients seeking enhanced functional vision without dysphotopsia.
