Purpose

To study the clinical profile of cataract and visual outcome of cataract surgery along with complication profile in eyes with varying degrees of high axial myopia

Setting

Global prevalence of myopia is on the rise, and according to WHO it is expected to grow from 22% in2000-52%2050affecting people across all ages BlueMountainsEyeStudy and the Beaver Dam Eye Study- association between myopia and cataract Compared to ECCE.SICS, PE has been found safein the treatment of cataract in myopia- some studies have reported a high incidence of RD.Presenting cohort of patientsofhigh myopia to understand the effect of axial length on cataract type and complicationswith Sx

Methods

Retrospective analysis of 558 eyes of 446 patients undergoing cataract surgery with axial length > 26mm from Jan-Oct2023 at a tertiary eye care institute were included. Three groups(Group A- 26 mm to >/= 28mm, Group B- >28mm to >/= 30mm, Group C- >30 mm). Baseline characteristics (age, gender along with clinical profile such as type and Catraact grade) noted. Presenting BCVA, refraction, axial length, keratometry, lens thickness, anterior chamber depth was noted. Preexisting ocular pathologies or history of surgeries were noted. Those eyes with no known ocular interventionincluded in outcome analysis.type of cataract surgery, intraandpostoperative complications/intervention final BCVA at last followup compared across groups and analyzed.

Results

Mean age-48.9y(253 M193 F).Bilateral in 113 patients. Nuclear cataract-371(66.48 %) and PSC -70 (30.46%). Pre-existing glaucoma-11 eyesretinal pathologies-86 eyes428 eyes included in outcome analysis.Mean age-GpA(50.8 ±16.1y) \GpB(47.4 ±15.6y) and GpC(46.1 ±12.6y).Mean postopBCVA-0.66 logMAR (GpA 0.62 logMAR,GpB 0.67 logMARGpC 0.74 logMAR BCVA) at last follow up. Univariate logistic regression analysis-no significant association of intraoperative complicationsin any group.Gp B-OR of 1.33 (95% CI 0.53–3.16 p = 0.528),GpC-OR of 1.69 (95% CI0.76–3.78 p = 0.195) for intraopcomplications logistic regression analysis evaluating secondary intervention-lower odds of requiring secondary intervention compared to GpA (OR0.55 95% CI0.29–0.97 p =0.045).

Conclusions

Patients with eyes with higher axial myopia tend to be younger. Cataract in these eyes is more commonly of nuclear variety. Cataract surgery in these eyes is not significantly associated with intraoperative or post-operative complications. These eyes achieve poorer best corrected visual acuity but do not need secondary intervention in most cases.
