Purpose

To compare visual outcomes in high myopia patients receiving wavelight plus versus Custom-Q FS-LASIK.

Setting

Peking University Third Hospital

Methods

This prospective, double-masked, comparative study enrolled high myopia patients (myopic ≥ 6D sphere) who received wavelight plus or Custom-Q FS-LASIK for both eyes, and the data of right eyes were included into analysis. Follow-ups were conducted at 1 day, 1 week, 1 month, and 3 months postoperatively. Visual acuity, manifest refraction, and higher-order aberrations (HOAs) were measured before and after surgery.

Results

There were 63 eyes of wavelight plus group and 54 eyes of Custom-Q group included into analysis. No significant difference was found in preoperative MRSE (-7.77±0.96 D vs -8.17±1.27 D, P=0.078) between groups. At 3 months postoperatively, the UDVA of 20/16 or better was measured in 88% of eyes in the wavelight plus group and 72% in the Custom-Q group (P=0.038). 95% in the wavelight plus group and 93% in the Custom-Q group achieved a postoperative MRSE within ±0.50 D. However, the wavelight plus group exhibited a slightly but statistically significant higher mean MRSE postoperatively (0.15±0.25 D vs. 0.03±0.32 D, P=0.032). Wavelight plus FS-LASIK exhibited a notable reduction in spherical aberration (SA) in both 4mm and 6mm (P<0.05) zone.

Conclusions

Both wavelight plus and Custom-Q FS-LASIK are highly effective in achieving excellent visual acuity and refractive predictability 3 months after surgery in patients with high myopia. Wavelight plus performed better in achieving UDVA and spherical aberration (SA) management.
