Purpose
Compare the results of vision quality after femtosecond laser-assisted in situ keratomileusis (FS-LASIK) for myopia and astigmatism (≥1.50D) using ray-tracing-guided (RT group) or Q-value-adjusted (QV group) methods.
Setting
Beijing AierIntecth Eye Hospital. Design: Retrospective cohort study.
Methods
Patients who underwent either ray-tracing-guided or Q-value-guided FS-LASIK were selected for this study. Postoperative evaluations included measurements of high-order aberrations, the Strehl ratio, and the modulation transfer function. A linear mixed model (LMM) was employed to compare the two groups, considering both binocular correlation and its differences. This study included 104 eyes in the RT group and 148 eyes in the QV group. Spherical equivalent for the RT group: -5.17 ± 1.85D; for the QV group: -5.18 ± 1.96D. Astigmatism for the RT group: -2.17 ± 0.59D; for the QV group: -2.17 ± 0.63D.The baseline characteristics of the two groups in age, UDVA, and CDVA were significantly different and were corrected for covariance.
Results
At 3-month postoperatively, CDVA, UDVA, SEQ, and cylinder were not significantly different (P=0.709, 0.173, 0.301, 0.251). Corneal aberrations at 4mm and 6mm included high-order aberrations, coma, total spherical (T.sph), and trefoil, which were mostly significantly lower (P<0.05) in the RT group, except for trefoil at 4mm, which was similar (P=0.256). Additionally, the RT group showed significantly lower corneal C12, spherical aberration (SA), and the Q-value for the 4mm, 5mm, and 6mm zones compared with the QV group (P=0.010, 0.042, < 0.001 for C12; < 0.001, < 0.001, < 0.001 for SA; < 0.001, < 0.001, < 0.001 for Q-value). No significant differences in the Strehl ratio and modulation transfer function (MTF) were found.
Conclusions
The application of Ray-tracing-guided and Q-value-adjusted FS-LASIK procedures for the correction of myopia and myopic astigmatism (≥1.50D) has been demonstrated to be safe and efficacious. Postoperative evaluations revealed that the Q value and high-order corneal aberrations—such as coma, trefoil, T.Sph, SA, and C12—were notably lower in the RT group compared to the QV group. The corneal spherical aberration remained the same or decreased in the RT group at 3 months, whereas it increased in the QV group.
