Purpose

To evaluate postoperative refractive outcomes and axis offset of a toric AddOn (1stQ Gmbh, Mannheim, Germany) Intraocular lens (IOL) after penetrating keratoplasty (PKP) and cataract surgery.

Setting

This single center, retrospective, explorative analysis was conducted at the Department of Ophthalmology
and Optometry at the Medical University of Vienna.

Methods

In this retrospective exploratory analysis, eleven eyes of 11 patients with a history of penetrating keratoplasty and subsequent cataract surgery underwent implantation of a toric AddOn intraocular lens.
Refractive outcomes were assessed by postoperative manifest refraction. Axis offset was evaluated using anterior segment optical coherence tomography (AS-OCT; Casia 2, Tomey Corporation, Japan) and postoperative ocular wavefront analysis was performed with a combined Scheimpflug camera/Placido disc topographer (Sirius) and a topo-aberrometer (Peramis) (SCHWIND eye-tech-solutions GmbH, Kleinostheim, Germany).

Results

Mean preoperative spherical equivalent (SE) and refractive cylinder (Cyl) were −2.10 ± 2.17 D [−4.88; 3.16] and +7.52 ± 2.07 D [3.25; 12.50], respectively. Postoperative mean SE was −0.53 ± 0.65 D and mean Cyl was +1.07 ± 1.16 D [0; 3]. The mean reduction in Cyl from pre- to postoperative was −6.45 ± 1.76 D [−9.50; −3.25]. Mean postoperative absolute IOL offset was 2.89 ± 1.27° [1; 5]. Mean higher-order aberrations (HOAs) at 5 mm were 1.58 ± 0.52 μm RMS [0.77; 2.38].

Conclusions

In this retrospective exploratory analysis, toric AddOn IOL implantation in eyes after penetrating keratoplasty demonstrated minimal axis offset compared to the target implantation axis. Postoperative refractive outcomes were satisfactory, with a mean reduction of 87% in refractive cylinder compared to preoperative values, as measured by manifest refraction.
