Purpose

To report postoperative outcomes in different cohorts of astigmatic subjects with antecedent corneal laser vision correction (LVC) surgery after they underwent cataract surgery with toric intraocular lens implantation (IOL) assisted with cloud-based surgical planning software (Veractiy, Carl Zeiss Meditec, Dublin, CA).

Setting

Retrospective comparative database study based on 326 cataract surgery cases performed on 167 subjects in the United States.

Methods

Inclusion criteria included eyes cataract surgery cases with astigmatism and prior corneal LVC surgery who underwent cataract surgery with toric IOL implantation with a target refraction in the IOL calculation of ± 0.25 D and for which preoperative biometry measurements performed using IOLMaster 700 and IOL calculation using Barrett True-K formula with measured anterior and posterior keratometry. Study endpoints were postoperative manifest astigmatism, spherical equivalent (SE) and uncorrected distance visual acuity (UDVA). Comparisons were made between monofocal versus presbyopia-correcting IOLs and between against-the-rule (ATR) and with-the-rule (WTR) preoperative corneal cylinder.

Results

Mean subject age included in this study was 67 ± 8 years, 52% were female. In the whole population, mean postoperative manifest astigmatism was 0.49 ± 0.45 D and 71% had astigmatism ≤ 0.50 D. Mean postoperative SE was -0.13 ± 0.55 D, and 79% had a SE of ± 0.50 D. No statistical difference was found between groups. Postoperative LogMAR UDVA was 0.05 ± 0.10 the whole population, 0.04 ± 0.10 in the monofcal group vs 0.06 ± 0.10 in presbyopia-correcting group (p<0.05), and 0.04 ± 0.10 in the ATR group vs 0.06 ± 0.09 in the WTR cohorts (p=0.12).

Conclusions

Astigmatic subjects with antecedent LVC who were implanted with a toric IOL following cataract surgery assisted by the Veracity surgery planner software experienced good postoperative refractive and visual outcomes. These results suggest that toric IOL implantation is safe and effective in this population.
