Purpose

To describe the outcomes of white mature cataracts

To describe the type of capsular rhexis pattern on the success of capsular rhexis in intumescent cataract.

To describe the reasons for delayed presentations for white mature cataracts in a peripheral set up of developing world country .

Setting

Patients with white mature cataracts presented to Base Hospital Kahawatta in Sabaragamuwa province Sri Lanka – A peripheral set up in a South Asian Country.

Surgeries were performed in pre described accepted standard techniques. No pre planned inerventions. All seurgeries were performed by a single surgeon.

Methods

Retrospectively analysed the outcomes of surgeries on white mature cataracts, no pre planned interventions done.

Through questionnaire analysed the details for delayed presentation for mature cataracts.

Results

51 patients with white mature cataracts were included. Age ranged from 42 years to 82 years. Mean age was 65.4 ± 9.6 years.

Preoperative vision all patients were 6/60 or less.

Best corrected post operative one-month distant visual acuity 38(74.5%) patients were 6/9 or better vision. Eventually 100% had vision 6/12 or better.

Capsular rhexis technique–Spiralling technique and double rhexis technique used. In spiralling method 2 rhexis extension in 21 cases.

In double rhexis technique no extensions in 30 cases.

Reasons for delayed presentation-Unawareness n=39 (76.5%), Fear to undergo a surgery n=3. Financial issues n=3, social issues preventing attendance to clinic n=4, medical co morbidities delaying presentation to eye care n=2.

Conclusions

Regarding the capsular rhexis in intumescent cataracts initial massaging of the anterior capsule and performing a two staged double rhexis – initial small rhexis, removing liquefied material and then performing a required size rhexis appears to be a safer option.

Visual outcomes are rewarding when compared to preoperative status however patients should be pre informed on the possibility of higher complications and possibility of co-existing posterior segment pathology unable to detect preoperatively due to media opacity.

When looking at the causes for delayed presentation people need more awareness, addressing the issue of fear of treatment and address the socio-economic issues.
