Purpose
To compare the patient reported outcomes with LUMINA® IOL with multifocal (MF) and EDOF IOLs using the Quality of Vision questionnaire (QoV, McAlinder©).
Setting
Vissum Miranza Alicante, Spain
Methods
This prospective comparative study included cataract patients implanted with LUMINA® IOL (AkkoLens) and MF or EDOF IOLs. The MF group included Vivinex Gemetric® (Hoya), Intensity® (Hanita) and AT LISA tri® (Zeiss); the EDOF group included AcrySof IQ Vivity® (Alcon), Asqelio EDOF® (AST) and TECNIS PureSee® (Johnson & Johnson). Follow-up at 1, 3, and 6 months assessed distance and near visual acuity, refraction, and ocular aberrometry (Osiris, CSO©). PROMs were evaluated using the QoV questionnaire. Kruskal-Wallis test with Bonferroni post-hoc and Rasch scores converted to 0–100 scale.
Results
At 6 months, QoV domain scores differed significantly among IOL designs (frequency p=0.005; severity p=0.129; bothersome p=0.012). The LUMINA® IOL showed outcomes comparable to those of the EDOF IOLs (LUMINA®: frequency 28.86; severity 26.30; bothersome 22.18 vs EDOF:24.13; 21.07; 17.18) and better outcomes than MF IOLs (33.30; 26.15; 24.51). Bonferroni post-hoc tests showed higher frequency and degree of bothersome for MF IOLs versus EDOF IOLs (frequency=0.004; bothersome=0.009). For optical quality (PSF), although no statistically significant differences were observed, the accommodative IOL showed better results (0.2235±0.0902) compared with the EDOF IOLs (0.1971±0.0678) and the MF IOLs (0.2132±0.0876).
Conclusions
LUMINA® IOL exhibited better clinical outcomes in terms of frequency and bothersome compared with multifocal IOLs and similar with EDOF IOLs, making it a valuable option for patients seeking to minimize postoperative photic phenomena. Multifocal IOLs demonstrated a higher subjective perception of photic phenomena in frequency and bothersome compared with EDOF IOLs. These findings underscore the importance of considering subjective visual quality when individualizing IOL selection.
