Purpose

This study aimed to assess contrast sensitivity and halo perception in a cohort of subjects younger than 40 years using the Real Artificial Lens Vision (RALV) simulator. Measurements were obtained with the natural lens and with six different multifocal intraocular lenses (mIOLs).

Setting

Prospective, randomized, single-masked study conducted at the Department of Ophthalmology of the University Medical Center Hamburg-Eppendorf, Germany.

Methods

The RALV simulator enables preoperative objective quantification of intraocular lens design-dependent optical phenomena, including contrast sensitivity (CS) and halo formation. Six mIOLs were examined:
1stQ Liberty² 3,00 (1stQ, Germany); Alcon Clareon PanOptix 3,25 (Alcon, USA); Teleon Lentis MF30 3,00 (Teleon, Germany); Zeiss AT Lisa tri 3,33 (Zeiss, Germany); Max Vision 2,80 (Ophthalmopro, Germany); Rayner Galaxy 3,00 (Rayner, United Kingdom). Baseline CS and halo measurements with the natural lens were compared with mIOL. Inclusion criteria were age ≥19years natural crystalline lens, BCVA ≥20/25 Snellen, sphere within ±4,0 diopters (D), cylinder within ±2,0D, no vision-impairing ocular diseases.

Results

In total, 35 eyes of subjects with a mean age of 28 years (SD: 4,6, range 19 to 40) were included. Mean CS for the natural lens was 1,44logCS Weber. The highest mean CS was measured for the Rayner Galaxy (1,29logCS W), the lowest for the Panoptix, Lisa Tri and Max Vision mIOLs (1,13 logCS W). Halos were significantly more pronounced with the mIOLs (14 mrad) compared to the natural lens (7,3 mrad). Lowest mean halo was measured for the Rayner Galaxy (10 mrad), while the highest mean halo was recorded for the Lisa Tri (16 mrad). The Liberty 2, Teleon and Max Vision mIOL had mean halo values of 13, 15 and 14, respectively.

Conclusions

RALV enables preoperative visualization of intraocular lens performance and objective quantification of mIOL-associated photic phenomena, enabling assessment of design-dependent trade-offs. Relative to the natural lens, mIOLs showed reduced contrast sensitivity and increased halos. Preoperative simulation may support patient-centered intraocular lens selection, particularly in refractive lens exchange and phakic intraocular lens implantation.
