Purpose

To quantitatively assess contrast sensitivity and halo perception of six multifocal intraocular lenses (mIOLs) using the real-time artificial lens simulator RALV in presbyopic patients.

Setting

This prospective, randomized study was conducted at the Department of Ophthalmology of the University Medical Center Hamburg-Eppendorf, Hamburg, Germany.

Methods

RALV allows patients to experience the visual impression of mIOls, including halos and contrast sensitivity (CS), prior to surgery. Six mIOLs were evaluated: Liberty2 (1stQ, Germany), Lisa Tri2 (Zeiss, Germany), PanOptix (Alcon, USA), LentisMF30 (Teleon, Netherlands), Max Vision (Ophthalmo Pro, Germany), Galaxy (Rayner, UK). A zero lens, which neutralizes the natural lens properties, was used to establish baseline measurements of CS for statistical comparison. For each subject, baseline measurements of CS, CS (log CS Weber), halos (in mrad) were assessed. Inclusion criteria: natural lens, age ≥ 40years, BCVA of ≥ 20/25 Snellen (ft), sphere within ± 4.0 diopters (D), cylinder within ±2.0 D and no vision-impairing ocular diseases.

Results

In total, 30 eyes of 30 subjects (mean age: 55 yrs, range 40-60) were included. The mean CS for all mIOLs was 0,9 logCS W, the mean of halos was 15,9 mrad. Significant reduction in CS was found for the mIOLs and the extent of CS reduction varied markedly. The highest mean CS was observed for the Lentis MF30 (1.1 logCS W) while PanOptix showed the lowest mean CS (0.87 logCS W). Compared to the zero lens, the Max Vision had the greatest reduction in mean CS (0.31 logCS W), the fewest decrease was seen for  the Galaxy (0.2 logCS W). Significant differences in halo size were observed with the lowest and greatest mean halo seen for the Galaxy (13.7 mrad) and Lisa Tri 2, respectively.

Conclusions

The RALV simulator is a valuable tool for prospectively assessing mIOL properties prior to surgery, potentially aiding in IOL selection. Quantitative trade-offs associated with specific mIOLs, such as the magnitude of CS reduction and halo size, demonstrably varied between the lenses tested.
