PAEDIATRIC OCULAR TRAUMA -A RETROSPECTIVE STUDY

Purpose

To Analyse the surgical outcome in Paediatric ocular trauma from western India

Setting

Tertiary care hospital in western India

Methods

In this retrospective study case files of all children less than 16 years of age presenting with either open globe or closed globe injury between 2014 January to December 2018 were retrieved from a Electronic data base patient demographics, mode of trauma, interval between injury and presentation, clinical manifestations, visual acuity, surgical interventions and outcomes were recorded and statistically analysed

Results

We included 78 eyes of 78 children with mean age of 7+- 1.5 years of which 43 boys(55%) open globe injury was seen in 52eyes(66.6%),of which 32(61.5%)limited to zone 1 mean visual acuity was 3/60(1.4 log Mar)by snellens chart. All patients underwent corned-scleral tear repair with additional procedures if indicated(PPV 7 eyes)and cataract(30 eyes). After mean followup of 6months the mean BCVA improved to 6/36(Log Mar 0.8)by snellens(P<0.001). Eyes that required PPV and those which had cataract extraction with IOL in the same setting(N=12) had poor visual outcome

Conclusion

In paediatric ocular trauma open globe injury with posterior segment manifestations requiring PPV have poor outcome. Co-existing cataract in open globe injury which underwent IOL implantation in primary setting also showed suboptimal visual outcomes due to persistent inflammation. We recommend primary would repair with or with out cataract extraction without IOL placement in the primary setting to have best visual outcome
