Purpose

To compare the visual outcomes of moderate astigmatism correction between toric implantable collamer lens (TICL) and implantable collamer lens plus opposite clear corneal incision (OCCI).

Setting

Eye and ENT Hospital, Fudan University, Shanghai, China.

Methods

A prospective randomized study. 70 eyes of 70 myopic patients with moderate astigmatism were enrolled and divided into the TICL and OCCI groups. Refractive outcomes and vector analysis were evaluated preoperatively, and 1 month and 1 year postoperatively.

Results

At the postoperative 1 year visit, the mean uncorrected distance visual acuity (UDVA) and corrected distance visual acuity (CDVA) had significantly improved in both groups. The OCCI group showed a slight regression tendency at 1 year (correction index [CI]: 1.14 vs 0.87, P < 0.001), whereas the TICL group showed stable astigmatism correction (CI: 0.80 vs 0.83, P = .351). Approximately 92% of eyes in the TICL group had angle of error (AE) within ±15, while 79% of eyes in the OCCI group were within that range.

Conclusions

Both surgical techniques showed promising results in terms of safety and efficacy for the correction of myopia in patients with moderate astigmatism. However, TICL implantation achieved better predictability and stability than implantable collamer lens implantation with OCCI.
