Purpose

This study aims to evaluate and compare the effectiveness of astigmatism correction in Small Incision Lenticule Extraction (SMILE) using two different femtosecond laser platforms: VisuMax 500 and VisuMax 800.

Setting

A retrospective comparative analysis was conducted on 449 eyes that underwent SMILE surgery and B&VIIT Eye Center (Seoul, South Korea) between November 2023 and February 2024 and who came to clinic for postoperative 1 year eye check up. 

Methods

Eyes were classified based on preoperative astigmatism. Small astigmatism group ( < -1.5D) had 334 eyes and the high astigmatism group (≥ -1.5D) had 115 eyes. In each group, remnant astigmatism was assessed based on each laser platform. A total of 449 eyes were analyzed, with 83 eyes underwent SMILE surgery with VisuMax 800 and 366 eyes underwent SMILE with VisuMax 500. Laser platform was chosen by paitents' preferences. In small astigmatism group, 279 eyes were treated with VisuMax 500, while 55 eyes were treated with VisuMax 800. In high astigmatism group, 87 eyes underwent SMILE surgery with VisuMax 500 and 28 eyes underwent surgery with VisuMax 800.

Results

In the small astigmatism group, the mean residual astigmatism was -0.39 ± 0.26 D in VisuMax 500 and -0.46 ± 0.35 D in VisuMax 800, showing no statistically significant difference (p = 0.65). In the high astigmatism group, the mean residual astigmatism was -0.56 ± 0.29D in VisuMax 500 and -0.46 ± 0.18D in VisuMax 800, showing the VisuMax 800 demonstrating statistically significant lower residual astigmatism in postoperative 1 year (p < 0.05).

Conclusions

While both VisuMax 500 and VisuMax 800 showed comparable efficacy in correcting lower degree astigmatism, VisuMax 800 showed superior result with astigmatism higher than -1.5 D.
