Outcomes Of Smartsight Lenticule Extraction With A Nomogram To Improve Predictability

Purpose

To assess the efficacy, safety and quality of vision after SmartSight lenticule extraction for the treatment of myopia and myopic astigmatism and to validate the effectiveness of a nomogram to enhance predictability.

Setting

Private practice, London, UK

Methods

A retrospective study of 53 consecutive patients (106 eyes) undergoing SmartSight lenticular extraction for myopia and myopic astigmatism was performed. Patients were treated to correct distance acuity in both eyes. A nomogram was used to adjust the refraction target in a subgroup of eyes. Uncorrected distance visual acuity, subjective refraction and quality of vision were assessed. Patients were followed up at day one, week 1, month 3, month 6 and month 12.  

Results

Patients’ mean age was 32.8 years. 84.1% of patients achieved day one monocular uncorrected distance visual acuity (MUDVA) of 0.1 LogMAR or better. Mean post-operative MUDVA at 3 months was -0.09 (SD 0.09) LogMAR. Mean post-operative spherical equivalent (SEQ) was 0.14D (SD 0.59). 94% of all eyes were within 1.0D of target SEQ. When a nomogram was used to adjust target SEQ, 100% of eyes were within 1.0D of the target SEQ. Astigmatism of <0.5D was achieved in 93.7% of  eyes. The change in Snellen lines between preoperative corrected distance visual acuity and postoperative uncorrected distance visual acuity showed a gain of one or more lines in 25% of eyes. 3 eyes (2.7%) had anterior haze at 3 months. 1 eye (0.9%) required an enhancement. 

Conclusions

SmartSight treatment for correction of myopia and myopic astigmatism is safe and effective at 12 months follow-up. The precise and individualised design of a nomogram is a vital element to ensure higher predictability.
