Purpose
To evaluate the role of larger lenticules, achieved through the use of a 7.7 mm optical zone, to minimizing the risk of myopic regression and improving visual quality following SMILE surgery.
Setting
Single-center study involving twenty patients undergoing SMILE with customized parameters to assess the optical benefits of larger lenticules.
Methods
A case series of twenty patients with mild to moderate myopia underwent SMILE using a programmed optical zone of 7.7 mm and a cap diameter of 7.9 mm. Postoperative outcomes, including visual acuity, refractive result, and higher-order aberrations, were evaluated over a 3-month follow-up. Myopic regression was assessed through changes in spherical equivalent refraction (SEQ) and corneal topography.
Results
Patients treated with larger lenticules demonstrated great refractive stability and no patient showed visual disturbances on nyktometry. No patient complained about night-vision disturbances, such as halos and glare, due to the larger effective optical zone accommodating scotopic pupil size. All patients showed excellent refractive results with 100% achieving UCDVA of 1.0 Snellen
Conclusions
The use of larger lenticules in SMILE, facilitated by a 7.7 mm optical zone, is a safe procedure with great refractive results. This approach offers a valuable strategy for optimizing outcomes, particularly in patients with larger pupil sizes. These findings highlight the importance of customizing optical zone parameters in SMILE to maximize biomechanical stability and patient satisfaction.
