Purpose

Patient with non-pristine eyes may seek for spectacle freedom. This study evaluates visual acuity at different distances after implantation of EDOF IOL Alcon Acrysof® IQ Vivity® (DFTx15) in non-pristine eyes with epiretinal membrane after successful cataract surgery or refractive lens exchange surgery.

Setting

This is a single surgeon, single site case series follow-up study in Silmaasema Silmasairaala,Turku, Finland.  

Methods

The DFTx15 was implanted in 17 eyes of 9 patients with asymptomatic epiretinal membrane. Epiretinal membranes were found in 14 eyes in pre operative OCT imaging. Patients did not report distortions in pre operative check up. Toric IOL versions were used when applicable. Micromonovision was targeted. Barrett lens factor of 1.99 was used. Post-surgery visit was at 1 month, and yearly after. e main outcome parameters were LogMAR uncorrected and corrected visual acuity at distance (UDVA 6 m), intermediate (binocular UIVA 63 and 100 cm) and near (binocular UCNVA 40 cm). ETDRS type charts with rulers were used.

Results

Patients mean age was 63 years, 4 were females, pre operative corrected visual acuitys were logMar 0.00/0.04, right/left eye, with spherical recfractive error range -3.0 - +2.9D/-6.8 - +3.3D, mean near add was 2.2D, and mean implanted IOL power was 20.5D/21.3D. At one year, none had distance corrected spectacles. 5/9 did not need reading glasses. Binocular UDVA was logMar 0.00. UDVA was logMar 0.05/0.12. CDVA was logMar 0.01/-0.01. Mean anisometropia was 0.26D. UCNVA at 40cm was logMar 0.32. UIVA was logMar 0.09 at 63cm and logMar 0.00 at 100cm. No halos or distortions were reported.

Conclusions

EDOF IOL Alcon Acrysof® IQ Vivity® gave good uncorrected distance and intermediate visual acuitys in non-pristine eyes with epiretinal membrane. Even same patients managed without reading glasses at daily life. Ocular co-morbidies should not automatically rule out the use of premium IOLs.
