Billateral Sequential Acute Corneal Hydrops Complicated With Unilateral Corneal Perforation

Purpose

Review of current treatments of acute corneal hydrops in ectatic corneal disease and a case report of sequential acute billateral corneal hydrops complicated with corneal perforation.

Setting

University eye hospital, Ljubljana, Slovenia.

Methods

PubMed literature search and review of treatment of acute corneal hydrops in ectatic diseases. Interval to corneal clearing with different techniques and complications of individual techniques were compared.

Results

Conservative therapy as well as surgical therapy have been advised in acute corneal hydrops. Corneal compression suttures, intracameral gas injection, intracameral avtologous platelet-plasma injection and closure of defect with posterior corneal substitutes have been implemented. To date there is no difference in time to resolution of acute corneal hydrops between conservative and surgical approach.

Conclusions

Acute corneal hydrops is a rare occurrence in corneal ectatic disease and rarely complicated by corneal perforation. Conservative and surgical management have shown similar outcomes.
