Purpose

To retrospectively analyse the indications , surgical characteristics and postoperative outcomes in patients who have undergone artifical corneal endothelial implantation commercially known as EndoArt .

Setting

Retrospective observational study conducted at Frimley Health NHS Foundation Trust , Surrey , United Kingdom

Methods

The artificial corneal endothelial implantation (EndoArt) was performed in 8 eyes of 8 patients , who were deemed suitable as opposed to conventional endothelial corneal grafting , and the indications , pre procedure vision , pre and post operative central  corneal thickness (CCT) , intraoperative complications and post operative outcomes were retrospectively analysed from the electronic medical records of the patients .

Results

8 eyes were included in the study. The indications were corneal graft failure(5 cases),and pseudophakic bullous keratopathy(3 cases). The preop vision ranged from HM to 4/60.Mean visual improvement was 4 Snellen lines with 1 patient not improving ,with maximal improvement in an eye with failed Penetrating Keratoplasty(6/9). Mean no of anchoring sutures used was 2. 85-100% C3F8 gas fill was achieved in all cases.Mean CCT pre op was 833±333 microns and mean post op CCT was 600±623 microns.CCT reduction was noted in 5 cases.Main post op complications were implant detachment in 6 cases and raised IOP in 4 cases. The main indications for redo surgery were for rebubbling and resuturing in a 3 cases.Improvement in comfort was noted in 4 cases.

Conclusions

Artifical Corneal Endothelial implantation surgery (EndoArt) was found to have promising postoperative results especially in terms of vision improvement , CCT reduction and comfort . Endoart implantation may be considered as a suitable alternative to conventional endothelial keratoplasty  in selected eyes.
