Purpose

The aim of the study is to evaluate the new unique extended depth of focus (EDOF) toric optical system (Well Fusion Toric™) thought to deliver astigmatism correction and high quality of vision without spectacles. Mini WELL Toric® and Mini WELL PROXA Toric® intraocular lenses (IOLs) share the same patented non-diffractive platform based on distribution of spherical aberrations in the central part of the optics and aspherical monofocal design in the periphery. The binocular combination of Mini WELL Toric® and Mini WELL PROXA Toric® ensures full presbyopia and astigmatism correction thanks to their complementary optics.

Setting

Innovation and Medical Science, SIFI SpA, Catania, Italy

Methods

The new EDOF IOL Mini WELL PROXA Toric® has a number of zones higher than Mini WELL Toric®, the active zone is extended up to 4.5 mm and spherical aberrations distribution is optimized to extend vision up to 3D. With optical bench testing, the characteristic of Mini WELL PROXA Toric® were compared to its complementary optic Mini WELL Toric®: IOLs optical performance, related to the EDOF aspheric design to the image quality, were tested to investigate depth of focus, modulation transfer function (MTF), straylight analysis and sensitivity to tilt, decentration and angle kappa.

Results

The MTF through focus (TF-MTF) curves as the retinal acuity charts showed a complementary performance of the IOLs from far to near while preserving a continuous contrast for all vision conditions. MTF values at 50 lp/mm for a 3-mm pupil were not influenced by tilt (±5°), by decentration (±1 mm), or by average human angle kappa. Straylight analysis confirmed the absence of dysphotopsia phenomena and the superior benefit of the Well Fusion Toric™ system.

Conclusions

The toric optical system, Well Fusion Toric™, was evaluated based on numerical simulations and experimentally characterized on an optical bench. The Mini WELL PROXA Toric® and Mini WELL Toric® provide a continuous focus from far to near through their high complementarity optics, closing the gap in near vision (30-35 cm) typically associated with EDOF technology. Well Fusion Toric™ represents an innovative solution for presbyopia and astigmatism treatment with uninterrupted high-quality vision at all distances.
