Purpose

To identify the anterior segment parameters related to vault at 1 month postoperatively, and establish a predictive formula for Implantable Collamer Lens (ICL) model selection based on ciliary sulcus-to-sulcus distance. 

Setting

Bidirectional study. In the formula - development group, a total of 114 eyes from 57 subjects (16 males and 41 females) were included aged 18-49 years (25.79±7.46). In the formula - validation group, there were 33 eyes from 18 subjects (5 males and 13 females) aged 18 - 41 years (26.67±7.87).

Methods

Standardized preoperative assessments included: uncorrected visual acuity, best-corrected visual acuity, biometry (IOL Master: WTW, AL, LT; Pentacam: Kf, Ks, ACD, ACV, ACA), and ultrasound biomicroscopy (H/V STS). ICL implantation followed STAAR Nomogram. Vault measurements were obtained via anterior segment optical coherence tomography at 1-month postoperative. The correlation between the vault and each preoperative parameter was calculated. Multivariable regression identified vault predictors. The validation group used mean absolute error (MAE), root mean square error (RMSE), and Bland-Altman plots to assess the accuracy of the prediction formula.

Results

Significant vault determinants included: corneal refractive power in the flat meridian, corneal refractive power in the steep meridian, horizontal corneal diameter, axial length, horizontal/vertical sulcus-to-sulcus distancesand, phakic intraocular lens size (P < 0.001). The final regression model JL1 :Vault (μm) = 633.0 + 1281.4 × (ICL size) - 1385.8 × HSTS (R² = 0.983, adjusted R² = 0.979). Validation showed MAE=127.1 μm and RMSE=150.3 μm. The Bland-Altman plot demonstrated that the majority of the scatter points were essentially within the range of the 95% confidence interval.

Conclusions
