Purpose

To analyze the clinical effects of patients with small eyes(Corneal diameter<10.80mm) after ICL and TICL V4C implantation. Let patients who have a small eye cannot undergo corneal laser surgery to choose ICL V4C with more confidence.

Setting

In China, the consensus of  experts points out that the corneal diameter needs to be greater than 10.80mm in ICL implantation. However,In clinical work, we often encounter some myopic patients with small corneal diameter who are very eager to undergo ICL surgery. After fully communicating with the patient about the risks of Inappropriate lens size or high intraocular pressure after surgery, can the patient choose ICL implantation?

Methods

Retrospective clinical study. Patients who underwent ICL or TICL surgery in our hospital from 2022 to 2023 were collected.Inclusion criteria: white to white <10.80mm, age 18 to 45 years old, completed 1-year follow-up period.The preoperative and postoperative visual acuity, refractive status, intraocular pressure, and corneal endothelial cell count were collected for statistical analysis, and the vault at each postoperative review was recorded.

Results

A total of 61 eyes with  the corneal diameter(white to white) less than 10.80mm underwent ICL or TICL surgery and completed the 1-year follow-up period.There are 3 male eyes.The WTW was 10.30 to 10.70mm. The ACD was 2.34 to 3.27mm .All eyes were implanted with 12.1mm lens.One year after surgery, 80% of eyes had UCVA that was 1 line or more higher than the BCVA before surgery, and the effectiveness index was 1.21.The postoperative vault ranged from 130μm to 543μm, with an average of 381μm.No replacement of lens due to inappropriate vault.The postoperative IOP and number of endothelial cells  was 14.88±3.64mmHg and 2888±345cell/mm2 ,which was not significantly different from the preoperative 14.79±3.69mmHg and 2888±321 cell/mm2 (P>0.05).

Conclusions

Among myopic patients, Female patients are more likely to have small eyes. ICL V4C implantation remains highly safe and effective in patients with small eyes
