Purpose

The pupil measured with modern pupillometers represents the entrance pupil rather than the exit pupil. The real pupil undergoes magnification influenced by anterior chamber depth and corneal curvature. This study aims to assess the mesopic pupil image before and after Intraocular posterior chamber lenses (ICL) implantation. Understanding the real pupil size in relation to ICL implantation is crucial, as the pupillometry may affect diffraction phenomena post-surgery and unwanted photic phenomenas.

Methods

This retrospective cohort study included 131 eyes that underwent ICL implantation. The collected data included age, simulated keratometry (SimK), pre- and post-operative anterior chamber depth (ACD), white to white (WTW), pre- and post-operative mesopic entrance pupil (Pm) at one year. Statistical analysis was conducted using SPSS software. The exit postoperative pupil size at the iris plane was estimated using a modified version of the Bennett and Rabbett formula (Clin. Vis. Opt. 1989), adapted for phakic eyes with ICL implantation.

Results

The mean values and standard deviations were as follows: age = 32 ± 5.33 years; pre- and post-operative ACD = 3.70 ± 0.27 mm and 3.02 ± 0.26 mm, respectively (p<.01); Kavg = 43.82 ± 1.57 D; WTW= 12.26 ± 0.48 mm. Preoperative and postoperative entrance Pm were 3.70 ± 0.73 mm and 3.62 ± 0.59 mm, respectively (p<.01), while exit Pm was 3.24 ± 0.63 mm preoperatively and 3.25 ± 0.52 mm postoperatively (p=.082). So, the magnification of pupil decreased from 14 ± 0.01% preoperatively to 11 ± 0.01% postoperatively (p<0.1), due to ACD change caused by vaulting of ICL.

Conclusions

Knowing the size of the entrance pupil is not sufficient. Since ICLs are lenses implanted posterior to the iris plane, it is necessary to measure or estimate the exit pupil (which corresponds to the real pupil). Finally, we did not observe any significant differences between the preoperative and postoperative mesopic exit pupil after ICL implantation.
