Three-Month Clinical Outcomes To Correct Myopia Or Myopic Astigmatism Using A Femtosecond Laser For Lenticule Creation With Schwind Atos

Purpose
To evaluate the safety and efficacy of lenticule creation and extraction using the SCHWIND ATOS femtosecond laser system with SmartSight for the refractive correction of myopia and myopic astigmatism.
Setting
Research Department at Ofttalmosalud, Instituto de Ojos, Lima, Peru.
Methods
A prospective observational study of myopic and myopic astigmatism patients undergoing refractive correction with SCHWIND ATOS using SmartSight. Patients over 18 years old with a corneal thickness ≥500 µm, residual stromal thickness >300 µm, and cap thickness >130 µm were included. Exclusions: abnormal corneal topography, herpes zoster ophthalmicus, herpes simplex, keratitis, resistant dry eye, diabetes, connective tissue diseases, pregnancy, or breastfeeding. Uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA), subjective refraction and biomicroscopy were assessed preoperatively and at 1, 7, 30 and 90 days postoperatively.
Results
A total of 49 eyes from 25 patients with a mean age of 28.5 ± 8.0 years were evaluated. The mean preoperative UDVA was 0.67 ± 0.78 LogMAR, while at 3 months postoperatively, it improved to 0.06 ± 0.09 LogMAR (p=0.0001). The preoperative CDVA had a mean of -0.02 ± 0.05 LogMAR, and at 3 months postoperatively, it was -0.01 ± 0.04 LogMAR (p=0.108). The preoperative spherical equivalent (SE) was -3.30 ± 1.33 diopters (D), and at 3 months postoperatively, it was -0.32 ± 0.42 D (p<0.001). Regarding efficacy, 80% of eyes achieved a UDVA ≤ 0.10 LogMAR, and 100% of eyes had a UDVA within 1 line of their CDVA. In terms of safety, no significant intraoperative complications were reported.
Conclusions
The SCHWIND ATOS femtosecond laser system with SmartSight for the refractive correction of myopia and myopic astigmatism proved to be safe and effective, improving UDVA in all cases of this study, with no evidence of ectasia post-lenticule extraction.
