Purpose
To investigate the value of CP combined with anterior segment OCT in the early diagnosis of keratoconus.
Setting
Thirty-three keratoconus patients (66 eyes) who visited our hospital from 2021 to 2023 were selected as the experimental group, and normal myopic patients who underwent refractive surgery in our hospital during the same period were selected as the control group
Methods
The data of Corvis biomechanical index (CBI), corneal topography and biomechanical index (TBI) were collected. Corneal topography Belin/Ambrosio comprehensive deviation (BAD - D) and at For the preclinical patients, the corneal epithelial thickness parameters were measured by anterior segment OCT in the range of 7mm pupil center diameter, the average corneal epithelial thickness values of 25 zones and the average corneal full thickness thickness values in the range of 9mm pupil center diameter. The differences of these parameters between the early keratoconus group and the control group were compared. The diagnostic value of the parameters with differences was analyzed
Results
Compared with the normal population, the clinical and subclinical periods of CBI,TBI, and BADD were statistically significant. The diagnostic efficiency of CBI, TBI and BADD for clinical keratoconus and subclinical keratoconus was as follows: BAD-D >TBI >CBI.The analysis of corneal epithelial thickness measured by OCT in the anterior segment showed statistical significance in the Std.Dev ,(Max-e)-(Min-e),Min-e,temporal, inferior, inferior nasal regions of the inner ring,and inferior nasal of middle ring.ROC curve analysis showed that Std.Dev value,(Max-e)-(Min-e) value,inferior, inferior nasal regions of the inner ring,and inferior nasal of middle ring had higher diagnostic efficiency

The measurement of corneal biomechanical index and corneal epithelial thickness has guiding significance for the diagnosis of subclinical keratoconus and preoperative screening of corneal refractive surgery.
