Purpose

To decscribe a new way to choose a Light Adjustable Lens IOL in patients with a previous history of radial keratotomy surgery.

Setting

Unviersity of Colorado Sue Anschutz-Rodgers Eye Center, Aurora, CO USA

Methods

We performed cataract surgery using the Light Adjustable Lens (Rx Sight) in 10 eyes of 5 patients who all have a history of radial keratotomy surgery. Instead of aiming for plano, we aimed for a myopic correction to aid in the post-operative hyperopia that would exist with a plano target. This is esepcially helpful since the final lock-in for LAL patients post-RK can take 1-2 months. There were concerns from the manufacturer (Rx Sight) about correcting these eyes from a myopic to hyperopic direction, however, all patients had excellent outcomes and all post-op targets were met.

Results

All patients had excellent post-op outcomes and all refractive targets were met.

Conclusions

The Light Adjustable Lens is an excellent option for patients with previous redial keratotomy surgery. Because these patients take a long time to correct post-operatively (1-2 months) we feel that a myopic target at the time of surgery will allow the patients to avoid being hyperopic in the post-op period which translates to better patient satisfaction without sacrificing refractive outcomes.
