Purpose

To evaluate the quality of vision, visual and refractive outcomes of patients undergoing unilateral cataract surgery with implantation of a rotationally asymmetric refractive multifocal IOL (Lentis Mplus, Teleon).

Setting

Vissum (Miranza Group), Alicante, Spain

Methods

A retrospective analysis was carried out at a single center, analyzing 26 eyes that had received hyperopic refractive surgery with the LENTIS Mplus multifocal IOL from January 2015 to December 2024. A comprehensive ophthalmologic exam was done before and after treatment at 1, 3, and 6 months. Visual and refractive outcomes for both far and near distances were registered. Quality of Vision (QoV) questionnaires assessed subjective visual function. The Wilcoxon signed rank test or student's t test will determine statistical significance of pre and post treatment differences, depending on the sample's normal distribution. All hypothesis tests will use 0.05 significance.

Results

UDVA, CDVA, UNVA, and CNVA improved significantly at six months and the final visit (0.10±0.14, 0.05±0.06, 0.27±0.22, 0.08±0.10 logMAR; p<0.05). Cylinder reduced to -0.38±0.46D (p<0.05), and SE stabilized at -0.00±0.70D. RMS values for total aberrations, astigmatism, coma, asymmetry, HOA, spherical-like, and coma-like aberrations were 0.59±0.36, 0.42±0.28, 0.20±0.12, 0.05±0.03, 0.39±0.28, 0.16±0.12, and 0.30±0.20. Defocus curve analysis showed excellent distance vision (0.00 logMAR at 0D) with smooth intermediate (-1.5D: 0.10±0.12) and near (-2.5D: 0.10±0.08) transitions. Contrast sensitivity remained stable. QoV scores were 32.97±24.31 for frequency, 30.47±22.19 for severity, and 28.59±21.32 for bothersomeness, indicating mild disturbances

Conclusions

After cataract surgery, the LENTIS Mplus multifocal intraocular lens (IOL) was able to successfully improve the visual and refractive results at far and near distance in patients who had previously undergone hyperopic refractive surgery.
