Purpose

To evaluate the quality of vision, visual and refractive outcomes of patients undergoing unilateral cataract surgery with implantation of a rotationally asymmetric refractive multifocal IOL (Lentis Mplus LS-313 MF30 or Lentis MplusX, Teleon).

Setting

Vissum (Miranza Group), Alicante, Spain

Methods

This retrospective study analyzed 39 unilateral cataract surgery patients implanted with the LENTIS Mplus multifocal IOL between January 2015 and December 2024 at a single center. Ophthalmologic exams were conducted preoperatively and at 1, 3, 6 months and last visit postoperatively. Visual and refractive outcomes for distance and near were recorded. Quality of Vision (QoV) questionnaires assessed subjective visual function. Pre- and postoperative differences were analyzed using the Wilcoxon test or Student’s t-test, with statistical significance set at p < 0.05.

Results

At six months, UDVA, CDVA, UNVA, and CNVA improved significantly (0.08±0.13, 0.01±0.08, 0.10±0.11, 0.05±0.07 logMAR; p<0.05). Cylinder reduced to -0.32±0.47D (p<0.05), and SE stabilized at -0.29±0.55D. RMS values for total aberrations, astigmatism, coma, asymmetry, HOA, spherical-like, and coma-like aberrations were 0.93±0.80, 0.73±0.72, 0.27±0.19, 0.15±0.16, 0.55±0.39, 0.30±0.20, and 0.45±0.34. Defocus curve analysis showed optimal distance vision (0.00±0.07 logMAR at 0D), with smooth intermediate (-1.5D: 0.12±0.07 logMAR) and near (-2.5D: 0.10±0.08 logMAR) transitions. Contrast sensitivity remained stable. QoV scores for frequency, severity, and bothersomeness were 26.61±19.31, 24.14±19.86, and 21.69±21.61, indicating mild disturbances.

Conclusions

Unilateral implantation of the multifocal IOL (Lentis Mplus LS-313 MF30 or Lentis MplusX, Teleon) is well tolerated and effective in improving visual function in unilateral cataract patients. It provides spectacle-independent vision from far to near, with high refractive predictability, stable optical quality, and minimal optical side effects.
