Clinical Features And Management Of Late Iol Decentration In The Dead Bag Syndrome

Purpose

To describe clinical features, demography, predisposing factors & surgical outcomes for late intraocular lens(IOL)decentration in dead bag syndrome.

Setting

Iladevi Cataract & IOL Research Centre, Raghudeep Eye Hospital, Ahmedabad, India

Methods

In this prospective interventional series, 46 eyes of 43 patients were enrolled.Main inclusion criterion was spontaneous posterior capsule rupture and IOL decentration/dislocation atleast 4 years after uneventful phacoemulsification surgery. All eyes had IOL exchange.Results included demography,risk factors,duration from primary cataract surgery, postop.visual acuity,complications.

Results

Study had distinct male preponderance.Axial myopia was a major risk factor. Mean duration between phacoemulsification and IOL exchange surgery was 16.4±5.5 years.Mean age at primary phacoemulsification was 52 years, & at IOL exchange was 67 years.Mean followup was 12.36 mths.One eye required a RD surgery,2 needed intravitreal injections for CME. Mean corrected distance visual acuity was 0.14 + 0.17 LogMAR units.

Conclusions

Dead bag syndrome is a distinct disease entity where capsular bag remains in situ but posterior capsule develops dehiscence,leading to partial/total decentration of IOL atleast 4 years following uneventful cataract surgery.Males & myopia are  main risk factors associated with this condition.
