Purpose

To present a case of a large limbal dermoid cyst in a 9-year-old male, managed successfully through simple excision, resulting in excellent cosmetic and visual outcomes.

Setting

University Clinic of Ophthalmology, Skopje, North Macedonia.

Report of case

A 9-year-old male presented with a temporal limbal mass measuring 11x4 mm, causing cosmetic concerns and discomfort but no visual impairment. Slit-lamp examination revealed a non-inflamed cyst with conjunctival and corneal involvement, containing 3-4 hairs. Preoperative visual acuity was 1.0 s.c. bilaterally. Diagnostic evaluations included orbital MRI and anterior segment OCT. Under general anesthesia, the conjunctiva was separated from the cyst, which was then excised from the cornea. The conjunctiva was sutured to achieve closure. The surgery was uncomplicated, and postoperative follow-ups at 1, 2, 3 months, and 1 year showed complete healing with satisfactory cosmetic results. Final visual acuity remained at 1.0 s.c., with no induced astigmatism or other complications observed.

Conclusion/Take home message

Simple excision of a large limbal dermoid cyst can be an effective surgical approach in pediatric patients, providing excellent cosmetic outcomes without compromising visual acuity.
