Restoring Sight In Aniridia With No Capsular Support: Case Series Of Iris Implant Techniques

Purpose

To present 4-cases of iris implant technique in aniridia eye with no capsular support.

Setting

We reported cases in Prof Ngoerah General Hospital, Bali, Indonesia.

Report of case

A 27-year-old male presented with left eye aphakia, aniridia, and vitrectomized eye after a motorcycle accident ten months prior. He complained of blurry vision and glare, with an uncorrected visual acuity (UCVA) of counting fingers at one meter. Lens implantation and pupillary reconstruction were performed using an Avansee™ three-piece IOL, which was implanted into a HumanOptics® artificial iris and sutured at four positions into the sclera. One month post-surgery, UCVA improved to 0.4 LogMAR and glare sensitivity reduced.
Two additional cases involved a 52-year-old female with right eye aphakia and iridodialysis and a 71-year-old male with right eye aphakia and aniridia after cataract surgery. Both had severe glare and blurry vision, with UCVA of hand movement. We utilized the Yamane technique to simultaneously implant an artificial iris along with the IOL, then securing the haptic IOL into the sclera.
The fourth case, a 48-year-old male with left eye aniridia and pseudophakia (intrascleral fixation), required only pupillary reconstruction using a HumanOptics® artificial iris implant, secured with three flange polypropylene of Yamane technique.

In all four cases, different iris implantation techniques showed an improvement in visual acuity, reduce glare, and enhance patient satisfaction for functional and cosmetic benefits. Mastery of various implantation techniques is crucial, as each case presents unique challenges.

Conclusion/Take home message

There are some techniques of iris implant surgery especially for aniridia with no capsular support cases.
