Purpose
To present Bilateral oil droplet pediatric cataract surgery with posterior Intraocular lens capture in a galactosemic patient
Setting
Ümraniye reseach and training hospital
Report of case
Case report; 5 year old female patient reffered to our clinic because of Bilateral oil droplet cataract. Systemic examination revealed galactosemia and mild mental retardation . Visual acuitiy cannot be evaluated . Intraocular pressure were 13/15 in both eyes and fundus examination were normal in both eyes.
Phacoemulsification surgery posterior capsulorexis and intraocular lens implantation were done with posterior optic capture. Postoperative refraction is +1.25(-1.75 130”) at right and +2.00(-1.75 130”) at left eye.
Discussion; Oil droplet Cataract Commonly seen in patients with galactosemia . Accumulation of dulcitol (galactose reduction product) within the lens cells leads to increased intracellular osmotic pressure and fluid influx into the lens.removal of galactose is may prevent cataract progression in early months but in toddlers cataract surgery is often needed .
Conclusion/Take home message
Early diagnosis of cataract in galactosemic patients is very important to prevent amblyopia
