Purpose

The purpose of this presentation is to introduce the concept of lens-based surgery for the treatment of keratoconus.

The recent introduction of small aperture intraocular lenses significantly impacted the treatment of irregular corneal astigmatism. The combination of pinhole optics with toric IOLs allows further improvement in outcomes.

Report of case

A 38y.o. man, with advanced keratoconus, presented with important complications related to the excessive use of scleral contact lens. Clear lensectomy was performed, with toric IOL implantation (ZCT800) combined with primary piggyback of the XtraFocus pinhole implant, achieving 20/40 J2 uncorrected.

Conclusion/Take home message

This case highlights the synergism between a high-cylinder toric IOL and the XtraFocus pinhole implant for the treatment of keratoconus.
