Purpose

To report a case in which an Add-On telescopic I O L (Sharioth Macular Lens, SML) was exchanged following faulty implantation technique and the lessons learned aftermath

Setting

A case report from dept. of cataract services in a tertiary eye hospital.

Report of case

A 70-year-old male with Dry AMD and geographic atrophy in the fovea underwent simultaneous cataract surgery with IOL and a +10D Add-On telescopic Sharioth Macular Lens (SML). After implantation of this special Add-On sulcus fixation IOL, it was a shocking surprise that two of the four haptics alongside a portion of the optic were broken and remained within the lens injector system, due to faults in the loading techniques. The incomplete Add-On IOL was then explanted and a fresh telescopic Add-On lens was implanted.

Conclusion/Take home message

SML is very delicate and non-forgiving. This is the first case report of the explantation of an Add-On Sharioth Macular Lens (SML). This case report also teaches a lesson on the ways to prevent such catastrophes.
