Outcomes Of Puresee Intraocular Lens Implantation In A Lasik Surgery Patient

Purpose

To report the refractive and functional outcomes of a patient with a history of LASIK surgery who subsequently underwent implantation of a presbyopia-correcting intraocular lens (TECNIS Puresee IOL).

Bayrampasa Eye Hospital,Istanbul, Türkiye

Report of case

A 64-year-old patient, who had undergone bilateral LASIK for myopia 15 years ago, presented with nuclear cataracts in both eyes during the diagnostic visit. The uncorrected visual acuity (UCVA) was 0.5 in the right eye (RE) and 0.05 in the left eye (LE). Refraction measurements were +0.50 -1.0X95 RE and -2.0 -1.50X90 LE. The best corrected visual acuity (BCVA) was 0.7 RE and 0.4 LE.

 Preoperative testing included topography, wavefront aberrometry using iTrace, biometry, endothelial cell count and macular OCT. The ASCRS IOL calculator was used for postoperative myopic refractive surgery adjustment. The surgery was performed according to standard cataract procedure with the implantation of a TECNIS Puresee 18.5D IOL without complications.

One month postoperatively, the manifest refraction was -0.25 -0.25 X50  RE and -0.50 -0.75 X100 LE. In binocular testing, UDVA was 1.0 and uncorrected near visual acuity was 1.0. Intermediate vision was checked at 60 cm 1.0. Preoperative total high-order aberration (HOA) was 0.158 RE and 0.267 LE. Postoperative total HOA was 0.126 RE and 0.172 LE. The patient reported very high satisfaction, with no complaints of halos or glare.

Conclusion/Take home message

This case demonstrates that the TECNIS Puresee IOL is effective improving visual performance in patients with a history of refractive surgery undergoing cataract surgery. Additionally, the patient reported high levels of satisfaction with no visual disturbances such as halos or glare. The TECNIS Puresee IOL provides a promising solution for presbyopia correction in patients with prior LASIK surgery.
