Bilateral Implantation Of The Ic-8 Small Aperture Intraocular Lens In A Post-Radial Keratotomy Patient: A Case Report

Purpose
To evaluate the visual outcomes and patient satisfaction following bilateral IC-8 Small Aperture intraocular lens (IOL) implantation in a patient with a history of radial keratotomy (RK).
Setting
Single-centre case study conducted in France, with postoperative follow-up assessments over 1-month.
Report of case
A 70-year-old patient with a history of RK (1984) underwent bilateral cataract surgery with IC-8 IOL implantation. Uncorrected and corrected monocular and binocular visual acuities were assessed at far, intermediate, and near distances. A bilateral defocus curve was also measured to evaluate the functional range of vision. The patient’s subjective satisfaction and reported postoperative comfort were recorded.
Postoperative visual outcomes were highly satisfactory, with the patient reporting no discomfort or pain. Uncorrected binocular visual acuities showed functional vision across all distances, with far at −0.10 logMAR. Uncorrected binocular near and intermediate visual acuities were maintained at 0.0 LogMAR. With correction, near and intermediate vision improved further. The defocus curve shows an extended depth of focus of 3D for a visual acuity better than 0.3 LogMAR. No adverse events were reported.
Conclusion/Take home message
The IC-8 Small Aperture IOL achieved highly satisfactory post-operative functional vision in a patient with a history of RK, achieving good depth of focus without reported adverse effects. This case suggests that small-aperture optics are an effective option for patients with complex corneal histories . Further studies with larger cohorts are needed to validate these findings.
