Osteo-Odonto-Keratoprosthesis Versus Tibial Bone Keratoprosthesis: A Systematic Review And Meta-Analysis

Purpose
Osteo-Odonto-Keratoprosthesis (OOKP) has been reserved for patients suffering from severe impairment in vision due to corneal clouding, while it is known that keratoplasty has a poor prognosis in those patients. We aimed to compare OOKP to the tibial bone keratoprosthesis in terms of anatomical and functional survival and postoperative complications.
Setting
A systematic review and meta-analysis.
Methods
Following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, we searched four electronic databases (PubMed, Scopus, Cochrane Library, and Web of Science) to identify eligible studies reported up to January 2024. Using STATA 17, we reported outcomes as risk ratios (RRs) or mean difference (MD) and confidence intervals (CIs). A P-value ≤ 0.05 is considered as statistically significant.
Results
OOKP was superior to tibial bone KP in terms of anatomical survival and functional survival after 10 years (log RR: 0.29 with 95% CI [0.17, 0.42], P < 0.001), and years (log RR: 0.73 with 95% CI [0.43, 1.03], P < 0.001), respectively. No significant difference in the post-operative complications including risk of infection, extrusion, vitreous hemorrhage, retinal detachment, mucosal graft necrosis, or endophthalmitis.
Conclusions
OOKP is associated with higher survival rates than tibial bone KP with no significant difference in the post-surgery complications.
