Purpose

Purpose: To evaluate visual outcomes, subjective and objective depth of focus curves, and aberrometry in patients with aberrated corneas implanted with the IC-8® small aperture (SA) intraocular lens (IOL) (Apthera™, Bausch & Lomb, Inc) compared to an aspheric monofocal IOL after cataract surgery.

Setting

Patients underwent surgery at 3 clinics in the USA.

Methods

This prospective observational study included 33 eyes with aberrated corneas (laser vision correction, RK, keratoconus) undergoing cataract surgery and implantation of the SA IOL in one eye and a monofocal IOL in the fellow eye. Postoperative assessments included uncorrected (UDVA), distance corrected (DCVA), intermediate (DCIVA), and near (DCNVA) visual acuities, subjective depth of focus (SDoF) curves, objective depth of focus (ODoF) from aberrometry-derived visual Strehl optical transfer function (VSOTF), and wavefront analysis (iTrace, Tracey technologies) in both groups. Corneal higher-order aberrations (HOAs) at 6-, 4-, and 2 mm at corneal plane aperture sizes were measured preoperatively using Pentacam Wave (Oculus Optikgeräte GmbH).

Results

The mean ± S.D. post-operative visions for the SA group (LogMAR) were: UDVA  0.14 ± 0.14, CDVA 0.05 ± 0.06, DCIVA 0.11 ± 0.11, and DCNVA  0.12 ± 0.11. The mean ± S.D. (*P ≤ 0.05) post-operative visions for the monofocal IOL group were: UDVA 0.10 ± 0.09 , CDVA 0.05 ± 0.05, DCIVA 0.25 ± 0.14*, and DCNVA 0.32 ± 0.17*.  The IC-8 group showed statistically significant better visual acuity in myopic defocus compared to the monofocal IOL group consistent with better near and intermediate visual acuities. Total RMS for the IC-8 group was 0.12 ± 0.07µ versus 0.48 ± 0.36µ for the monofocal group (p<0.002).

Conclusions

Depth of focus provided by small aperture optics was statistically significantly better than the monofocal IOL control eyes in patients with aberrated corneas. SDoF measurements correlated to the ODoF estimated from ray tracing aberrometer. Near visual acuity with the IC-8 were 3 to 4 lines better than the monofocal IOL group.
