In-The-Bag Behaviour Of A New Aspheric Hydrophilic Acrylic Toric Intraocular Lens

Purpose

To evaluate tilt, decentration and axial stability of the RayOne EMV toric intraocular lens (TIOL) over a 6 month follow up period.

Setting

This single center, prospective, interventional clinical trial was conducted at the Department of Ophthalmology
and Optometry at the Medical University of Vienna.

Methods

Sixty-eight eyes from 44 patients received a RayOne EMV TIOL.  Tilt, decentration and the aqueous depth (AQD) were determined preoperatively, at 1 hour (1h), 1 week (1w) and 6 months (6m) postoperatively using an anterior segment OCT Casia 2 (AS-OCT, Tomey Corporation, Nagoya, Japan). Difference vectors were used to assess changes in individual decentration and tilt. Tilt and decentration of the TIOL were calculated using the corneal vertex as a reference point.

Results

Mean tilt and decentration values were 5.2 ± 1.4° and 0.14 ± 0.11 mm for the crystalline lens, 5.4 ± 1.2° and 0.22 ± 0.10 mm for the TIOL at 1h, 5.1 ± 1.3° and 0.20 ± 0.10 mm at 1w and 5.2 ± 1.8° and 0.22 ± 0.10 mm for the TIOL at 6m. The highest tilt outlier was 9.7° at 6m. Mean individual changes in tilt  were 1.07 ± 1.10° from preoperative to 1h,  0.8° ± 0.9° from 1h to 1w, 1.23° ± 1.34° from 1w to 6m and 1.53° ± 1.63° from preoperative to 6m. The tilt difference vectors showed a significant difference between 1h to 1w and 1w to 6m  (p = 0.04). Mean individual changes in decentration from preoperative to 6m were 0.19 ± 0.11 mm. A posterior axial shift of 0.15 ± 0.18 mm was observed between 1h and 6m.  

Conclusions

The RayOne EMV TIOL demonstrates low tilt, decentration and satisfactory axial stability.  Difference vectors showed that the TIOL remained largely stable throughout the 6-month follow-up period. However the individual tilt of the TIOLs changed significantly more from 1w to 6m compared to the period from 1h to 1w, which could be attributed to the anterior fibrosis of the capsular bag.
