Purpose

This analysis aims to present refractive outcomes and the range of vision extension in clinical defocus curve analysis with the RayOne Galaxy Toric (Rayner, UK), a novel spiral full range of vision intraocular lens (IOL). This is the first fully toric subgroup analysis of the real-world multicentre evaluation

Methods

This observational multicentre case series includes data of 82 eyes (51 patients) implanted with the RayOne Galaxy Toric IOL. Postoperative data are available for 80 eyes at 1 month (1M) and 61 eyes at 3 months (3M). Refraction outcomes include manifest refraction reported in sphere, cylinder and mean refractive spherical equivalent (MRSE). Monocular distance corrected defocus curve measurements were performed in half dioptre steps from +1.0 D to -4.0 D at 1M and 3M after surgery. 

Results

Toric IOL with cylindric powers of 0.75 to 3.0D were implanted. The mean manifest cylinder of -1.03±0.81D (N=68 preoperatively) decreased to -0.25±0.29D at 1M (N=80), and -0.23±0.29D at 3M (N=61). The MRSE of 0.46±2.48 preoperatively decreased to -0.32±0.48D (1M) and -0.30±0.31D (3M). At 1M (n=42) and 3M (n=46) post-op the monocular distance corrected defocus curves showed a peak at 0 (distance) with a mean visual acuity (VA) of -0.02±0.07, 0.02±0.08 logMAR, respectively. At -1.5 D defocus (~66 cm) the mean VA were 0.08±0.11 and 0.08±0.09 logMAR, respectively. At a defocus of -3.0 D (~33 cm) the mean VA were 0.27±0.14 and 0.31±0.13 logMAR, respectively. A VA of 0.2 logMAR or better was achieved from +1.0 D to -2.5 D.

Conclusions

The refractive outcomes show a reduction in cylinder at 1M with stable results at 3M after surgery. The monocular defocus curves demonstrated stable defocus patterns at 1M and 3M follow-ups. The monocular defocus curves showed a full range of vision extension, with a smooth plateau from +0.5 D to -2.0 D, and VA of 0.2 logMAR or better across a ~3.5 D range. The results demonstrate that the RayOne Galaxy Toric full range of vision IOL featuring a unique spiral design provides a smooth and continuous range of focus from far to near distances. The results underline that RayOne Galaxy Toric is reliable option for patients with pre-existing corneal astigmatism wishing a high degree of spectacle independence.
