Myopic Shift Of Automated Refraction In A New Chromatic Aberration-Correcting Multifocal Intraocular Lens And Correlation With Ocular Biometric Measurements

Purpose
To evaluate the correlation between myopic shift in automated refraction after implantation of a new chromatic aberration-correcting multifocal intraocular lens (DRN00V, Johnson&Johnson Vision) and axial length, pupil size, and keratometry, and to compare of myopic shift between DRN00V and a previous chromatic aberration-correcting multifocal intraocular lens (DFR00V).

Methods
A retrospective comparative study enrolling 190 eyes undergoing cataract surgery with implantation of multifocal IOLs (DRN00V group 62 eyes, DFR00V group 128 eyes) were performed. Spherical equivalent of automated refraction (AR) utilizing TONOREFâ…ˇ, manifest refraction (MR), and best corrected distance visual acuity were evaluated 3 months postoperatively. K value, pupil diameter, and anterior chamber depth were measured postoperatively utilizing CASIA2.
Results
The automated refraction showed myopic shift compared to manifest refraction in all cases in DRN00V group postoperatively. The average AR was -1.34±0.47D, MR was -0.21±0.29D, and myopic shift (AR-MR) was -1.13±-0.36D. The number of eyes with myopic shift from -0.25D to -0.5D, from -0.5D to -1.0D, from -1.0D to -1.5D, from -1.5D to -2.0D, and over -2.0D were 3, 24, 30, 4, and 1 eyes respectively (4.8%, 38.7%, 48.4%, 6.5%, 1.6%). The automated refraction of DRN00V group was significantly more myopic compared to DFR00V group (Turkey-Kramer, p<0.05). The amount of myopic shift in DRN00V group statistically correlated with axial length (p<0.05), but did not correlate with K value, pupil diameter and anterior chamber depth.
Conclusions
The amount of myopic shift of automated refraction was significantly higher in the DRN00V group compared to DFR00V group, which implies DRN00V has more chromatic aberration-correction than DFR00V. Manifest refraction measurement after implantation of DRN00V should be performed carefully in order to compensate for myopic shift by automated refraction, especially in long axial length cases.
