Raytracing Lasik (Wavelight Plus) Vs. Small Lenticle Extraction (Smile) For Myopia Correction: A Randomized, Prospective, Contralateral Eye Study

Purpose
Purpose: This study evaluates safety, efficacy and visual outcomes of Ray Tracing LASIK vs. Smile in a contralateral eye fashion.
Setting
Setting: The Laservision Clinical and Research Institute, Athens, Greece
Methods
Methods: This contralateral eye study included 120 eyes of 60 patients: one eye was treated with RayTracing LASIK (wavelight plus. Alcon/Wavelight, Erlagen Germany), while the fellow eye was treated with SMILE (SMILE pro, Visumax 800, Zeiss, Jena, Germany)) using the RayTracing calculated refraction (SightMap, Alcon/Wavelight, Erlagen Germany). The following parameters were evaluated preoperatively and up to 3 months postoperatively: uncorrected distance vision acuity (UDVA), corrected distance vision acuity (CDVA), refractive error, corneal tomography, spherical equivalent (SE), defocus equivalent (DE), high order aberrations (HOA): total: RMSh and spherical aberration, and contrast sensitivity,
Results
Results: At 3 months, 100% of the RayTracing LASIK group and 100% of the SMILE group had UDVA of 20/20, 100% and 85%, respectively, had UDVA of 20/16, and 50% vs. 4% had UDVA 20/12.5 (P < 0.01). SE (±0.25 D) was 78.5% for the LASIK group and 52.3% for the SMILE group. DE: (up to 0.5D) 100% and 85 % respectively. Residual tomographic cylinder (> 0.50 D) was 15.8% for the LASIK group and 52% for the SMILE group (P < 0.01). High order aberrations: mean RMSh 0.21 um vs 0.37 um respectively. In regard to the 3-month spherical aberration measured the comparison was 0.053 vs. 0.067 um, respectively (P=0.04)
Conclusions
Conclusions: RayTracing LASIK and SMILE Pro are both lamellar femtosecond laser assisted laser refractive corneal procedures. This 3-month comparison of the two techniques in a randomized contralateral eye study demonstrated excellent efficacy and safetyfor both techniques. RayTracing LASIK appears to offer superior visual outcomes in lines of vision gained, defocus equivalent, mesopic and scotopic contrast sensitivity, high order aberrations including spherical aberration.
